Adding Life Care Management to Your Life Care Planning Practice
There is a growing need for professionals to provide services to older adults, individuals with chronic health conditions & disability. These services include care planning, coordination of care, comprehensive assessments, ongoing management, advocacy, concierge-style support, transition assistance, and advisory services. Individuals, families, attorneys, insurers and other stakeholders increasingly rely upon experienced and capable professionals to help provide solutions and answers while helping to navigate the often complicated healthcare & long term care industry. Life Care Planners make great Life Care Managers because of their consistent and reliable methodology, experience working with a myriad of complex health conditions while understanding the current needs, and working collaboratively to help determine the probability for future care designed for optimal health & wellness.
